Assertive Community Treatment at Lawren Healthcare Services is a highly intensive, community-based program designed for adults with serious and persistent mental illness who have significant difficulty engaging with traditional clinic-based services. Rather than requiring clients to come to us, the ACT team comes directly to them — in their homes, neighborhoods, shelters, community centers, or wherever they are. The ACT model is built around a dedicated multidisciplinary team — including a psychiatrist, nurse, licensed therapist, case manager, peer specialist, and vocational counselor — who work together to provide comprehensive, coordinated care to a small caseload of individuals with complex needs.
